Job summary

Optum Home & Community Care, part of UnitedHealth Group, is hiring a Telephone Case Manager RN for remote work. The role supports a diverse member population with education, advocacy and connections to resources to stay well at home.

The HouseCalls Clinical Support Team provides telephonic consultation post HouseCalls visits to ensure smooth transition of care back to the member's Primary Care Provider. Team includes nurse care managers and social workers.

Qualifications

• Undergraduate Degree required.
• Current, unrestricted Compact RN license in the state; additional licenses within 6 months may be required.
• 3+ years of clinical RN experience in hospital, acute care, home health/hospice, direct care, or case management.
• 1+ years remote call center experience.
• Ability to work at home in a call center environment that requires being on the phone with members/providers for the majority of the shift.

Responsibilities

• Perform telephonic outreach to members identified by the Advance Practice Clinician for specific referral related issues.
• Ensure member has scheduled appointment with PCP/specialty provider; assist with scheduling if needed.
• Refer members to internal departments such as Social Work or Clinical Help Desk when appropriate.
• Gather clinical information telephonically from patient/family.
• Assist patients/members with urgent needs requiring acute intervention that arise during the call.
• Identify triggers for hospitalization and barriers to meeting healthcare goals during the call.
• Complete required documentation in compliance with auditing standards and policies.
• Provide patient/family education on disease process and trigger management.
