Job summary

CVS Health is seeking an experienced Registered Nurse Case Manager to deliver telephonic case management for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members navigate resources, improve health outcomes, and overcome barriers to care.

This remote, full-time role offers a Monday–Friday schedule with daytime CST hours and no nights, weekends, or holidays.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work within Monday-Friday business hours.
• Preferred: Case Management Certification ( CCM or equivalent ).
• BSN preferred.

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to clinical programs and resources.
• Apply clinical judgment to support members with complex and co‑morbid conditions.
• Collaborate with multidisciplinary teams to achieve optimal member outcomes.
• Maintain compliance with regulatory requirements and company policies.
