Optum Home & Community Care, part of the Optum family, is transforming care delivery. The RN Case Manager – HouseCalls works remotely, providing education, advocacy and coordination for members with an EST or CST schedule and a compact RN license.

In a telephonic call center setting, you’ll help schedule appointments, connect patients with services, document care interactions, and collaborate with clinicians to ensure a smooth transition of care for patients staying at home.

Qualifications

• Undergraduate Degree required.
• Current, unrestricted Compact RN license in the state.
• 3+ years of clinical experience as an RN in hospital, acute care, home health/hospice, direct care, or case management.
• 1+ years remote call center experience.
• Ability to work from home in a HIPAA-compliant, dedicated workspace with high-speed Internet.

Responsibilities

• Perform telephonic outreach to members identified by the Advance Practice Clinician for referral-related issues.
• Ensure member has scheduled appointment with PCP or specialty provider; assist with scheduling if needed.
• Refer members to internal departments (Social Work, Clinical Help Desk) as appropriate.
• Gather clinical information telephonically from patient/family.
• Provide patient education on disease processes and trigger management.
• Document all interactions in electronic clinical records per policy.
• Maintain HIPAA confidentiality and comply with auditing standards.
