Job summary

CVS Health is seeking a Field Case Manager RN in Passaic County, NJ. This role assesses, plans, implements, and coordinates case management activities for members with a focus on dual eligible populations, in a full-time schedule from 8:00 am to 5:00 pm.

The ideal candidate holds an Associate's Degree in Nursing (Bachelor's preferred), with 3+ years clinical practice and 2+ years in care management or related areas. NJ RN license required; CCM preferred.

Qualifications

• Associate's Degree in Nursing required.
• Bachelor's Degree in Nursing preferred.
• Minimum 3+ years of clinical practice experience.
• Minimum 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination preferred.
• Active and unrestricted RN license in the state of NJ required.
• Certified Case Manager preferred.

Responsibilities

• Assess, plan, implement, and coordinate all case management activities with members.
• Develop a proactive plan of care to enhance short and long-term outcomes.
• Use clinical tools and data review to evaluate member needs and benefits.
• Apply clinical judgment to address complex health and social indicators.
• Conduct assessments considering various sources, including claims.
• Use a holistic approach to determine the need for referrals to clinical resources.
• Collaborate with supervisors and stakeholders to overcome barriers.
• Present cases at interdisciplinary case conferences.
• Utilize motivational interviewing skills for member engagement.
• Ensure compliance with regulatory and company policies and procedures.
