Qualifications

Responsibilities

• Provide in-person and telephonic Medical Case Management to patients, families, providers, and employers.
• Assess, plan, implement, and evaluate patient progress and treatment plans.
• Travel locally up to ~60% of time and may require overnight trips; conduct home visits.
