Job summary

You will collaborate with physicians, families, and employers, assess progress, plan care, and ensure medical necessity and cost effectiveness. Regular local travel up to 60% is required, with standard business hours and opportunities for professional growth.

Qualifications

• RN license active in Oregon.
• Graduate of accredited school of nursing.
• CCM or CIRS certification preferred.
• Experience as an RN Case Manager ideal.
• Willingness to travel locally up to 60%.

Responsibilities

• Provides in-person and telephonic Medical Case Management to patients, physician, employer and referral source.
• Discuss treatment plans with physicians and explore alternate plans.
• Assess, plan, implement and evaluate patient progress.
• Evaluate treatment need and cost effectiveness.
• Attend doctors' visits and discharge planning conferences.
• Coordinate delivery of durable medical equipment and nursing services.
• Travel locally up to ~60% of the work week/month.
