Qualifications

• Registered nurse with current RN licensure in the operating state.
• Experience as an RN Medical Case Manager preferred; CCM/CIRS certifications preferred.
• Graduated from an accredited school of nursing.

Responsibilities

• Provide in-person and telephonic Medical Case Management to patients, families, and providers.
• Discuss treatment plans with physicians and adjust as needed.
• Assess, plan, implement, and evaluate patient progress.
• Coordinate with discharge planning and arrange durable medical equipment.
• Travel locally up to 60% of the work week; overnight travel possible.
• Attend doctors/other providers' visits and conferences.
