Job summary

CVS Health is seeking a Registered Nurse Case Manager for a field/remote role serving Essex County, NJ, and surrounding areas. The position involves up to 75% travel, with a Mon-Fri 8am-5pm EST schedule and significant home-visit responsibilities.

Qualifications

• Minimum associate’s degree or diploma in Nursing required.
• 3+ years clinical practice experience in a hospital or alternative care setting.
• Active and Unrestricted RN license in NJ required.
• Successful completion of the NJ Choice certification is a contingency of employment.

Responsibilities

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate medical needs of members enrolled in MLTSS and D-SNP/FIDE programs.
• Facilitate members' wellness by addressing requests for services.
• Schedule and attend interdisciplinary meetings advocating safe discharge.
• Develop care plans and authorize services within MLTSS/FIDE benefits.
• Document accurately and timely in the member’s electronic health record.
• Coordinate care with the member, authorized representative, and care team.
• Mentor new hires once proficiency is attained.
