Job summary

Support, Inc. is seeking a part-time or full-time nurse to coordinate medical care for individuals with developmental disabilities in Colorado. You will work with physicians, teams, individuals, and families to plan, implement, and monitor health services and treatment options.

The role emphasizes advocacy, care planning, and collaboration across teams to ensure quality outcomes. Remote-friendly with occasional travel to front-range offices and client homes.

Qualifications

• RN with Colorado license is required.
• Experience with IDD populations is preferred.
• Nurse case management experience is a plus.

Responsibilities

• Provide delegation and consultation to unlicensed staff, families, and the organization.
• Offer medical consultation to interdisciplinary teams.
• Teach and train staff in QMAP, MRB, and medical documentation.
