Shine Residential seeks an experienced RN Case Manager to coordinate medical care for individuals with developmental disabilities in home and community settings. You will assess health needs, manage medications, and work with families, DSPs, and regional centers to implement individualized plans.

You will travel to client homes, conduct assessments, provide hands-on care, and ensure compliant, patient-centered care that improves quality of life.

Formación

• Active, unrestricted RN license in California
• Graduate of an accredited nursing program (ADN or BSN)
• Minimum of two years RN experience, including at least one year in case management, community health, home health, or with individuals with intellectual/developmental disabilities
• Working knowledge of Lanterman Developmental Disabilities Services Act, California CCR Titles 17 & 22, regional center system and IPP process, and HIPAA requirements
• Demonstrated ability to conduct health and functional assessments, develop care plans, manage medications, and escalate changes in condition
• Ability to teach and coach direct support professionals on health, safety, and medication procedures
• Strong clinical documentation skills in EMR and MS Office/Google Workspace
• Excellent communication and collaboration with clients, families, physicians, and community partners

Responsabilidades

• Evaluate health and functional status of clients in homes or shared housing.
• Provide hands-on nursing care in clients' homes and travel to conduct visits.
• Coordinate IPP goals with clients, families, DSPs, regional center staff, and physicians.
• Educate clients and families about care plans, disease management, medications, and safety.
• Oversee medication regimens and train staff on administration and documentation.
• Provide clinical guidance and health-safety training to DSPs and program managers.
• Act as escalation point for medical emergencies and hospital-to-home transitions.
• Advocate for clients' rights and coordinate accommodations with providers.
