Nursing and Therapy Services of Colorado (NTSOC) is seeking a Field Nurse Case Manager to oversee admissions and recertification assessments, coordinate patient care, and supervise field staff. You will collaborate with the multi-disciplinary team to educate staff and ensure compliance with all policies and regulations.

The role combines remote work with on-site patient visits in southern Colorado. Requires Colorado RN license and nursing experience; BSN preferred.

Qualifikationen

• Graduate of an Accredited Nursing Program required.
• BSN preferred.

• Provide comprehensive assessments to determine eligibility and continued eligibility.
• Coordinate and manage patient care with the care team to ensure timely documentation.
• Implement physician orders accurately and manage patient care visits.
• Ensure compliance with agency policies and state/federal regulations.
