Support, Inc. is seeking a part-time or full-time Nurse to coordinate medical care for individuals with developmental disabilities in Colorado. The role involves collaboration with physicians, interdisciplinary teams, and families to assess health needs and implement care plans.

Qualified applicants are RNs with a current Colorado license; preferred experience includes IDD care and nurse case management. Occasional travel to offices and client homes is required; a personal vehicle is necessary.

Formación

• RN with current Colorado license.
• Preferred experience with individuals with IDD.
• Nurse case management experience.

Responsabilidades

• Provides delegation and consultation to unlicensed providers, families, and Support Inc staff.
• Provides medical consultation to interdisciplinary team members.
• Teaches and trains providers, families, and staff in: Medication Administration (QMAP) and MRB training; Individual specific medical documentation; Evaluate signs of illness or dysfunction that warrant medical or nursing intervention; Basic skills required to meet the health needs of the individual; Utilizing specialized medical equipment.
• Applies knowledge of nursing practices, state and federal regulations, guidelines, and procedures to ensure compliance with home and community-based services.
• Completes and documents evaluations and assessments to identify training and support needs to ensure continuity of care.
• Develops care plans, reviews medical documentation, and health tracking.
• Develop effective working relations and cooperate across teams throughout the entire case management process.
• Help individuals receiving services and their caretakers make informed decisions by acting as their advocate regarding their clinical status and treatment options.
