Job summary

HealthPartners is seeking an RN Case Manager to support patients, families, and physicians in addressing medical and social concerns and to educate patients for informed health care decisions. This role is primarily remote with several in-person member visits weekly in homes, healthcare facilities, and community settings around the Metro area.

Direct accountability to the Case Management team with a growing caseload of up to 85 members.

Qualifications

• RN with Minnesota license (unrestricted) and BSN preferred.
• Minimum 3 years clinical practice in acute care.
• 3+ years in utilization review, discharge planning, or case management.

Responsibilities

• Focus on member-centered care and education to support personal health goals.
• Integrate clinical and psychosocial information for care planning and implementation.
• Communicate with patients, families, providers, and health plan to enable collaboration.
• Maintain documentation and confidentiality per policy and procedure.
• Foster teamwork and participate in departmental and organizational meetings.
• Remain proficient with automated systems and identify training needs.
