Rocky Mountain Health Care Services (RMHCS) in Colorado Springs seeks an RN Home Care Coordinator to join the interdisciplinary team. You will present nursing interventions for care planning, report changes in condition, and help manage patient care across home health services.

The role includes chart audits, on-call coverage, and direct interaction with participants and families. Strong communication and organization are essential to support delivery of high-quality home care.

Qualifications

• RN license in Colorado is required.
• Minimum one year of healthcare experience preferred.
• CPR and First Aid certification preferred.
• Proficient in Microsoft Office and electronic medical records.

Responsibilities

• Serves as a member of the IDT.
• Presents nursing interventions for care planning and reports changes in condition.
• Performs chart audits and ensures nursing documentation is complete.
• Supports home care nurses with procedures, documentation, and task organization.
• Performs home visits to administer medications and treatments.
• Covers after-hours and weekend on-call rotation as needed.
• Documents interactions in the EMR.
• Educates participants and families on medications and plans of care.
