Job summary

Openkrill is seeking a Registered Nurse Case Manager to collaborate with clinical teams and coordinate care for patients with chronic or serious conditions. You will advocate for personalized treatment, provide education, and help navigate complex medical decisions within a value-based care model.

Qualifications

• Licensed RN with clinical care management experience.
• ACM certification preferred.
• 5 years’ RN or RN Case Manager experience in complex care management.

Responsibilities

• Engage patients needing clinical support to assess and prioritize needs; provide patient-centered care and education.
• Coordinate telephonic outreach to identified patients and ensure information for home transition is clear.
• Collaborate with PCPs/NPs and health teams to keep patients stable at home and coordinate services.
