Job summary

CVS Health is seeking a Case Manager - Registered Nurse to assess, plan, and coordinate care for members across the care continuum, including home visits and telephonic interactions. This remote, Ohio-based role supports individuals and families in achieving optimal health outcomes.

Qualifications

• RN degree or nursing diploma required.
• Bachelor's degree preferred.
• Active RN license in OH or state of residence.

Responsibilities

• Use a collaborative process for assessment, planning, and care coordination.
• Act as a liaison with members, families, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases and chronically ill members.
• Interact with members telephonically or in person, including home visits.
• Assess and analyze medical and vocational status of members.
• Develop plans of care to optimize wellness and medical outcomes.
• Communicate with stakeholders regarding member care.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams to maximize health outcomes.
• Monitor member progress toward desired outcomes.
