Job summary

Sagility is seeking an experienced Registered Nurse for a Senior Care Specialist role that is work-from-home. You will conduct telephonic assessments, develop care plans, and coordinate transitions of care for Medicare/Medicaid and group health plan members.

This remote role requires strong clinical judgment, independent work capability, and excellent communication skills. The position offers a seasonal arrangement with starting pay stated per hour, and daily pay options.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, provider appointments, and other resources.
• Help members and caregivers navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors that may affect a member's health and well-being.
• Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote member self-management by providing education and resources appropriate to individual health conditions and goals.
• Coordinate care and condition-management activities among members, caregivers, providers, and other members of the care team.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately and in a timely manner within applicable systems.
