Sagility is seeking a Senior Care Specialist (Registered Nurse) to support Medicare/Medicaid and group health plan members from home. You will conduct telephonic health assessments, develop individualized care plans, and coach members to manage chronic conditions.

This remote, seasonal role offers flexible scheduling and the chance to collaborate with a multidisciplinary team to improve transitions of care, medication management, and access to benefits.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficient with Microsoft Word, Excel, and Outlook.

Responsabilités

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME and home health services.
• Help members navigate healthcare benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams to optimize care.
• Promote self-management with education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions and outcomes accurately in systems.
