Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) to perform work-from-home health assessments and care coordination for Medicare/Medicaid and group plans. The role emphasizes remote nursing judgment, care planning, and multidisciplinary collaboration.

Ideal candidates have 2+ years of clinical nursing with RN licensure, compact state authorization, and strong communication skills to educate and support members navigating complex health needs.

Competenze

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Mansioni

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, and appointments.
