Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. The role focuses on telephonic health assessments, care planning, health coaching, and coordination of care for Medicare, Medicaid, and group health plan members.

The position is seasonal, with an anticipated end date in June 2027. You will collaborate with a multidisciplinary team and operate in a remote setting across the United States.

Competenze

• Active, unrestricted RN license in resident state.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum 2 years clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical thinking, problem-solving.
• Ability to work independently in a remote environment.
• Proficient in MS Word, Excel, Outlook; navigate multiple systems and EMRs.
• Strong verbal and written communication.

• Conduct telephonic health assessments for members.
