Job summary

Sagility is hiring a Senior Care Specialist (Registered Nurse) for a work-from-home role in the United States. The RN will perform telephonic health assessments, develop individualized care plans, and coordinate care with a multidisciplinary team to support Medicare/Medicaid members.

This seasonal opportunity emphasizes remote work and requires strong clinical judgment. Starting pay is $37.00 per hour, with benefits like daily pay access.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum 2 years of clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical-thinking and problem-solving.
• Ability to work independently in a remote environment.
• Proficiency with Microsoft Word, Excel and Outlook.

Responsibilities

• Conduct telephonic health assessments for members.
• Develop individualized care plans based on health risks.
• Establish health coaching relationships and provide education.
• Use nursing judgment to assess needs and coordinate interventions.
• Support transitions of care and discharge planning.
• Help navigate benefits and care options for members.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Document interactions and outcomes accurately in systems.
