Job summary

Prominence Health is seeking an RN Care Manager to provide care management services for medically and/or socially complex members. You will develop comprehensive care plans, assess needs, and coach for adherence to plans, including end-of-life planning.

This role focuses on culturally sensitive care coordination, collaboration with contracted physicians, and partnerships with community resources to improve outcomes and manage costs. Location: TX, remote within TX.

Qualifications

• Associate or Bachelor’s Degree in Nursing, required.
• Active, unrestricted Compact RN license in the States of Practice (Nevada, Texas and/or Florida), required, multi-state preferred.
• Certified Case Manager (CCM), Case Management Nurse – Board Certified (CMGT-BC), Accredited Case Manager – RN (ACM-RN), or Certified Managed Care Nurse (CMCN), preferred.

Responsibilities

• Provide care management services for medically and/or socially complex members.
• Develop comprehensive care plans for member and family self-care competence.
• Assess needs and monitor a specific individualized care plan, including end of life planning.
• Promote knowledge of the Care Management program to Prominence Health Plan contracted physicians and members.
• Develop and sustain partnerships with community resources and support agencies.
• Contribute to successful SNP CM program implementation and ensure model of care meets regulatory requirements.
