Qualifications

• RN license in TX/NV/FL (compact preferred).
• Minimum 3 years clinical nursing experience.
• Minimum 3 years in Case Management/Transition of Care.

Responsibilities

• Provide care management services for medically and socially complex members.
• Develop comprehensive care plans and coach for adherence to the plan.
• Promote the Care Management program to contracted physicians and members.
• Develop and sustain partnerships with community resources and support agencies.
