Job summary

Prominence Health seeks an RN Care Manager located in Texas to provide care management for medically and socially complex members. You will develop personalized care plans, coach for adherence, and coordinate with contracted physicians and community resources to improve outcomes.

Under CMSA Standards of Practice, you will participate in SNP CM program activities and ensure model of care meets regulatory requirements. Remote in TX and strong English/Spanish communication are favored.

Qualifications

• Active, unrestricted Compact RN license in TX, NV, or FL; multi-state preferred.
• 3+ years in clinical nursing practice.
• 3+ years in Case Management/Transition of Care in managed care setting.

Responsibilities

• Provide care management services for medically and/or socially complex members.
• Develop and monitor comprehensive care plans including end-of-life planning.
• Coordinate with physicians, community resources, and support agencies.
