Qualifikationen

• Active RN license required in WA/OR/AK
• CCM credential preferred
• 3+ years clinical practice in hospital, clinic, home care or nursing facility preferred
• Experience with electronic health records or health registries required

• Develop and coordinate individualized care plans with physicians and care teams
• Coordinate access to health services across multiple providers and disciplines
• Monitor outcomes and cost drivers and adjust care plans as needed
