Job summary

Centene is seeking a Senior Care Manager (RN) to assess, plan, and implement complex care management activities that enable quality, cost‑effective healthcare outcomes. This role is ideal for experienced nurses who excel in communication and managing complex medical caseloads.

Location: Remote, OK with residency in Oklahoma. This is a full-time, day shift position requiring an RN license and 4–6 years of related experience.

Qualifications

• Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.
• RN licensure required.

Responsibilities

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify needed resources for members.
• Coordinate care between members, families, and care provider teams.
• Monitor care plans and member status, revising as necessary.
• Identify problems/barriers for care management in escalated cases.
• Review member data to identify trends and improve care quality.
• Develop appropriate care plans/service plans from referrals and assessments.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain member information for compliance.
• Provide education to members and their families on healthcare options.
