Job summary

Molina Healthcare is hiring a remote RN Care Manager to support Medicare members through telephonic case management and care coordination. Preference for Washington state, PST hours, with no travel or nights.

Ideal candidate has at least 2 years experience in care management within an MCO, active RN license, and strong communication. Emphasizes collaboration with interdisciplinary teams and HIPAA/EMR compliance.

Qualifications

• At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience.
• RN license must be active and unrestricted in state of practice.
• Valid and unrestricted driver\'s license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and be detail-oriented.
• Ability to work in diverse settings and with various populations.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.
• Proficiency with Microsoft Office and online portals/databases.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plan with member, caregiver, physician and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts goals as needed.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina guidance to support change.
• Assesses barriers to care and provides care coordination to address concerns.
• May provide consultation and resources to peers as needed.
• Care managers may handle complex cases and medication reconciliation.
• Travel 25-40% may be required depending on state/contractual requirements.
