Job summary

Molina Healthcare is seeking a remote RN Care Manager to support Medicare members through telephonic case management and transition-of-care functions. The role favors candidates in Washington state and operates on Pacific Standard Time hours.

Responsibilities include comprehensive assessments, care coordination plans, and collaboration with a multidisciplinary team. Travel up to 25–40% may be required depending on state/contract.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• Registered Nurse (RN) license active and unrestricted.
• Valid driver’s license and reliable transportation.
• Understanding of EMR systems and HIPAA.
• Knowledge of community resources and how to access them.
• Ability to work independently and manage multiple priorities.
• Strong verbal and written communication skills.
• Experience with diverse populations and settings.

Responsibilities

• Completes comprehensive member assessments within regulatory timelines.
• Develops and implements care coordination plan with member, caregiver, physician and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors and documents progress toward goals; suggests changes as needed.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Assesses barriers to care and provides care coordination assistance.
• May provide consultation to peers.
• Care manager RNs may handle complex member cases and medications; medication reconciliation as needed.
• 25-40% travel may be required.
