Qualifications

• Bachelor's degree in nursing or equivalent required.
• Registered Nurse license in state of residence.
• Minimum 5 years of relevant clinical experience.
• Strong communication and motivational interviewing skills.
• Experience in home care or case management preferred.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Assess member needs, goals, and barriers to wellness to develop care plans.
• Develop and revise member-specific care plans and oversee care manager interventions.
• Document in applicable platform per policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate smooth transitions of care and post-discharge follow-up to prevent readmissions.
• Collaborate with interdisciplinary team to improve member outcomes.
