Job summary

IntelliResume is seeking a Care Manager (RN) to develop, assess, and facilitate complex care management activities for members with physical needs. This role emphasizes high-quality, cost-effective outcomes and collaboration with providers across a person-centered care team.

Qualifications

Responsibilities

• Evaluate the needs of the member and recommend a plan for the best outcome.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate interventions.
• Coordinate between the member and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and care plan goals.
• Provide resource support for local services based on assessments.
• Facilitate care management and ensure timely access to needed services.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain member information for compliance.
