Qualifications

Responsibilities

• Develop and continuously assess care plans.
• Collaborate with providers to identify resources for members.
• Monitor member status and revise plans as needed.
• Identify problems and barriers in care management.
• Review data to identify trends and improve quality.
• Develop care plans based on referrals and assessments.
• Perform telephonic, digital, and home outreach to assess needs.
• Collaborate with healthcare providers to facilitate services.
• Document and maintain member information for compliance.
• Educate members and families on healthcare options.
