Job summary

Centene Management Company LLC is seeking a registered nurse to join our care management team. The role focuses on developing, assessing, and facilitating care plans for members with complex physical needs to improve outcomes.

The schedule is Monday through Friday, 8-5 PM CT or ET, with preference for candidates in eastern or central time zones and holding an active Kentucky RN license. Remote/hybrid options may be available.

Qualifications

• Requires a degree in nursing or BSN and 2–4 years of related experience.
• Active Kentucky State or Compact RN license required.
• Experience in medical-surgical, inpatient, home health, or care management settings.

Responsibilities

• Evaluates member needs and barriers to care, guiding personalized plans.
• Develops and updates care/service plans with providers.
• Coordinates between member, family, and care team to ensure appropriate services.
• Provides ongoing follow-up and monitors progress toward goals.
• Educates members and families on disease processes and care options.
• Documents all activities per regulatory requirements.
