Job summary

You will monitor member progress, adapt plans, and ensure timely access to services. Travel is approximately 10% for meetings and team gatherings, with a remote-friendly schedule for residents of North Carolina.

Qualifications

• Requires RN license in North Carolina.
• Bachelor's degree in Nursing or equivalent with 2–4 years of related experience.

Responsibilities

• Develops, assesses, and facilitates complex care management activities for primarily physical needs members.
• Creates and updates care plans; collaborates with providers to identify resources and specialists.
• Coordinates between member, family/caregivers, and care team to ensure person-centered care.
• Provides education to members and families on disease processes, care options and benefits.
• Monitors member status and revises care plans as needed to meet goals.
• Collects and maintains member information to comply with state, federal and payer regulations.
