Centene Corp. is seeking a Remote Care Manager (RN) in Kentucky to develop and manage individualized, cost-effective care plans for members with complex needs. You will coordinate with providers, educate families, and monitor progress to ensure high-quality outcomes.

The role requires an active RN license (Kentucky or compact), 2–4 years of relevant experience, and a focus on patient-centered care delivered via telephonic or digital outreach where appropriate.

Formación

• Requires RN license in Kentucky (or compact state)
• 2–4 years of related care management experience
• Experience in medical-surgical, inpatient, home health, or outpatient care management environments

Responsabilidades

• Develops and facilitates complex care management plans and service plans.
• Collaborates with providers to identify resources and address unmet needs.
• Provides education to members and families about disease processes, care options and benefits.
• Monitors member status, adjusts care plans, and follows up on outcomes.
• Coordinatesthe care between member, family, and care team to ensure person-centered care.
• Provides access to local resources and social determinants of health support; may include telephonic or home outreach.
