Job summary

Qualifications

• Degree in Nursing from an accredited program.
• RN licensure in state or compact state.
• 2–4 years of related experience in care management.

Responsibilities

• Evaluate needs and develop care plans for optimal outcomes.
• Collaborate with providers to address unmet needs.
• Identify barriers to care and intervene appropriately.
• Coordinate between member and care team for person-centered care.
• Provide follow-up, monitoring, and education to members and families.
