Qualifications

• Requires an active New Jersey RN license.
• Bachelor's degree in Nursing or equivalent from an accredited school.
• 2–4 years of related experience.

Responsibilities

• Evaluates member needs and barriers to care to plan outcomes.
• Develops care/service plans and coordinates with providers.
• Identifies barriers and provides care management interventions.
• Coordinates between member/family and care team for person-centered care.
• Provides follow-up and monitors status; updates care plan as needed.
• Provides resources for local services based on assessments.
• Facilitate care management and ensure timely access to services.
• May perform telephonic, digital, home, or site outreach.
• Collects and maintains all member information to ensure regulatory compliance.
• Educates members and families on disease processes, care options, and benefits.
• Provides feedback to leadership to improve care delivery.
• Performs other duties as assigned.
• Complies with all policies and standards.
