Job summary

Qualifications

Responsibilities

• Develops and coordinates complex care management activities.
• Completes documentation in accordance with state, federal, and third-party payer requirements.
• Provides education to members and families about disease processes and care options and benefits.
• Collaborates with providers to identify resources and services to address unmet needs.
• Manages a caseload and coordinates member care with in-home visits up to 50% travel.
