Green Key Resources is seeking experienced remote Care Managers for the New York metro area. This role supports MLTC members by coordinating medical, psychosocial, and functional needs with an interdisciplinary team.

The ideal candidate is an RN licensed in NY with prior managed care or MLTC experience and is able to attend monthly meetings in Manhattan. Responsibilities include telephonic assessments, care planning, and connecting members with benefits and community resources to improve

Qualifications

• RN, NY State license is required.
• Experience in MLTC or managed care settings preferred.
• Ability to attend monthly meetings in Manhattan.

Responsibilities

• Perform assessments of members' medical, psychosocial, physical and spiritual needs.
• Develop, implement, and monitor person-centered service plans.
• Ensure continuity of care across healthcare settings by collaborating with the care team, PCP, and member representatives.
• Utilize benefit structure and community resources to meet members' needs.
• Educate members on disease processes, preventive health, and quality of life interventions.
• Complete initial telephonic assessments and review data per departmental workflows.
