Job summary

Prominence Health seeks a dedicated RN Care Manager who will provide essential care management services for members with complex medical needs. Under the guidance of the Care Management Clinical Program Manager, the RN will develop individualized care plans while promoting the Care Management program to both contracted physicians and health plan members.

The role requires effective communication skills and at least three years in clinical nursing practice.

Qualifications

• Active, unrestricted RN license in Florida, Nevada, or Texas is required.
• Minimum 3 years in clinical nursing practice.
• Minimum 3 years of Case Management experience preferred.
• Knowledge of Special Needs Plan population preferred.
• Bilingual in Spanish or other languages preferred based on location.

Responsibilities

• Develop comprehensive care plans for members.
• Assess member needs and coaching for adherence.
• Promote Care Management program to physicians and members.
• Participate in the implementation of the SNP CM program.

Medicare and Medicaid experience

Associate or Bachelor’s Degree in Nursing
