Job summary

Texas-licensed RN in Texas, you will perform telephonic care management for complex cases, assessing needs and coordinating services until discharge, within Molina Healthcares managed care framework.

You will collaborate with an interdisciplinary team, document care plans, monitor progress and support members with behavioral health and LTSS needs. Some travel (25-40%) may be required depending on contract.

Qualifications

• 2+ years in health care with care management or LTSS experience.
• RN license active and unrestricted in state of practice.
• Proficiency with Microsoft Office and ability to multitask across systems.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Develop and monitor care plans including waiver services.
• Coordinate with providers, caregivers, and members to achieve desired outcomes.
• Document interventions and maintain accurate contact notes for members.
• Facilitate care team meetings and coordinate LTSS services.
