Job summary

CenterWell Certified Healthcare Corp. is seeking an RN Care Manager to support Wisconsin Family Care members in the Wisconsin Rapids area with a 8:00 am – 4:30 pm CST schedule, no night or weekend shifts, and the ability to work remotely from home while traveling up to 40% to visit members.

You will assess health and safety needs, develop an MCP, coordinate with providers, educate on diseases and healthy lifestyle choices, document within DHS timelines, and conduct monthly phone contacts with

Qualifications

• Must have Associate degree in nursing.
• Wisconsin RN license in good standing.
• Residence within 45 mins (or less) of assigned coverage area.
• 1 year of care management experience preferred.

Responsibilities

• Assess members focusing on health and safety to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Participate in monthly member contacts and travel to in-person visits.
