Job summary

CenterWell Certified Healthcare Corp. is seeking a RN Care Manager to support Wisconsin Family Care members with care coordination, education, and support. The role does not include night or weekend shifts and involves travel to member homes approximately 40% of the time.

You will assess health needs, develop MCPs, educate on disease processes, and coordinate with primary and acute care providers. Work hours are typically 8:00 am–4:30 pm CST, Monday–Friday, with remote flexibility.

Qualifications

• RN degree or equivalent is required.
• Current Wisconsin RN license (in good standing) required.

Responsibilities

• Assess members to develop a comprehensive Member Care Plan.
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise, and lifestyle changes.
• Document in member files within required DHS timeframes.
• Participate in monthly member contacts and in-person visits.
• Work remotely from home and travel ~40% to visit members.
