Job summary

Inclusa is seeking an RN Care Manager to support Wisconsin Family Care members with comprehensive care coordination, education, and ongoing assessments. This full-time role is remote with travel up to 40% and standard hours Monday–Friday, 8:00 am–4:30 pm CST.

The successful candidate will have an Associate degree in nursing and Wisconsin RN licensure in good standing, with preferred 3 years in care management and a proximity within 45 minutes of the coverage area.

Qualifications

• Requires Associate degree in nursing and Wisconsin RN license.
• 3 years Care Management experience (preferred).
• Reside within 45 minutes of assigned coverage area.
• Active RN license in good standing in Wisconsin.

Responsibilities

• Assess members and develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments and coordinate with acute/ primary care providers.
• Educate members on diseases, nutrition, exercise and lifestyle changes.
• Document in member files within required DHS timeframes.
• Participate in monthly member contacts and travel to in-person visits (roughly 40%).
• Ensure proper handoff and task assignment in D365; support remediation efforts.
