Humana Inc. is seeking an RN Care Manager to support Wisconsin Family Care members, coordinating care and providing education while maintaining a work-life balance with no night or weekend shifts.

The role involves remote work from home with up to 40% travel to member sites in Wisconsin Rapids. You will assess health needs, develop MCPs, coordinate with providers, and educate on disease processes, nutrition, and lifestyle changes, documenting within required timelines.

Qualifications

• Associate degree in nursing required.
• Licensed Registered Nurse in Wisconsin in good standing.
• 1 year of Care Management experience preferred.
• Reside within 45 mins of assigned coverage area.
• Must be able to travel up to 40%.
• Typical schedule Monday–Friday, 8:00am–4:30pm CST.

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Participate in member monthly phone contacts and travel to attend in person visits in members setting.
