Author Health, LLC seeks a committed RN Care Manager to coordinate care for adults with behavioral health needs in a fully remote capacity. The role emphasizes collaboration with medical providers to improve outcomes, reduce readmissions, and ensure smooth transitions from facility to home.

The ideal candidate will have an active RN license in Texas or a multi‑state compact, 3+ years in behavioral health or medical case management, strong communication skills, and proficiency with EHR systems.

Qualifications

• Current RN licensure in Texas, Florida, or any multi‑state compact.
• At least 3 years of experience in behavioral health or medical case management.
• Strong understanding of behavioral health conditions, treatments, and evidence‑based practices.
• Excellent assessment, interpersonal, and communication skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Proficiency in electronic health records (EHR).

Responsibilities

• Perform comprehensive assessments of patients' behavioral health and medical needs.
• Remove barriers to engaging in health care services to ensure medication and treatment adherence.
• Create personalized care plans in collaboration with patients, their caregivers, and healthcare providers.
• Oversee and coordinate the execution of care plans to ensure they achieve the patients' goals and objectives.
• Perform comprehensive medication reconciliation by collecting and reviewing medication histories, identifying discrepancies, and ensuring accurate documentation to enhance patient safety during care transitions.
• Educate patients on health conditions, treatment options, and self-management strategies.
• Advocate for patients to ensure they access appropriate medical and behavioral health services in a timely manner.
• Work with interdisciplinary teams, including psychiatrists, nurse practitioners, licensed clinical social workers, and field workers to enhance patient care outcomes.
• Regularly monitor and evaluate patient progress, adjusting care plans as needed.
• Maintain precise and current documentation of patient interactions and interventions.
• Engage in case conferences, team meetings, and quality improvement initiatives.
• Participate in daily huddle meetings.
