Job summary

Centene Corporation is seeking a Behavioral Health Care Manager to develop and assess complex care plans for members with mental health and substance use needs. The role focuses on education, coordination, and cost-effective outcomes, with remote work options and a schedule of Monday–Friday, 8:00 am–5:00 pm PST.

WA state licensure or Compact RN may be required depending on contract. You will evaluate needs by phone or in-home visits, coordinate with providers, and monitor outcomes to ensure

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2–4 years of related experience.
• Licensed Masters Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience required.

Responsibilities

• Evaluate member needs via phone or in-home visits and develop care/service plans.
• Conduct outreach to assess needs and coordinate with resources.
• Develop ongoing care plans for high-acuity members and identify providers.
• Coordinate between member/family and care providers to ensure access to services.
• Monitor and adjust care plans based on treatment changes and outcomes.
• Facilitate care coordination to ensure timely access to care or services.
• Collect and maintain member information to meet regulatory requirements.
• Educate members and families on procedures, options, and benefits.
• Provide leadership feedback to improve care delivery.
• Perform other duties as assigned.
