Job summary

Author Health, LLC is seeking a Care Manager (RN) to coordinate care for adults with behavioral health needs in a fully remote role. You will partner with interdisciplinary teams to improve health outcomes, reduce readmissions, and ensure smooth transitions from facility to home.

The ideal candidate holds an active RN license in Texas (or a multi-state compact) with 3+ years in behavioral health or medical case management, and strong communication skills.

Qualifications

• Active RN license in TX or compact state.
• 3+ years in behavioral health or medical case management.
• Proficient in EHRs and care coordination workflows.
• Excellent written and verbal communication; patient-centered.
• Bilingual in Spanish/English advantageous.

Responsibilities

• Perform comprehensive assessments of behavioral health and medical needs.
• Coordinate care plans with patients, caregivers, and providers.
• Oversee execution of care plans to meet patient goals.
• Reconcile medications and review histories for safety.
• Educate patients on conditions, treatments, and self-management.
