Author Health is hiring a Care Manager (RN) for a fully remote role based in Florida. You will coordinate care for adults with behavioral health needs and medical treatment, aiming to improve outcomes and prevent readmissions.

The ideal candidate is a registered nurse with 3+ years in behavioral health or medical case management, strong communication, and proficiency with EHR systems. Remote work with minimal travel is supported.

Qualifications

• Current RN licensure in Texas, Florida, or any multi-state compact jurisdiction.
• At least 3 years of experience in behavioral health or medical case management.
• Strong understanding of behavioral health conditions, treatments, and evidence-based practices.
• Strong understanding of chronic medical conditions in adults that lead to ED/inpatient presentations and prevention strategies.
• Excellent assessment, interpersonal and communication skills.
• Ability to work independently and within a multidisciplinary team.
• Proficiency in EHR and healthcare technologies.
• Dedication to patient-centered care and advocacy.

Responsabilités

• Perform comprehensive assessments of patients' behavioral health and medical needs.
• Remove barriers to engaging in health care services to ensure medication and treatment adherence.
• Create personalized care plans with patients, caregivers, and providers.
• Oversee and coordinate the execution of care plans to achieve patient goals.
• Conduct comprehensive medication reconciliation to ensure safety during care transitions.
• Educate patients on health conditions, treatment options, and self-management strategies.
• Advocate for patients to access appropriate medical and behavioral health services promptly.
• Work with psychiatrists, NPs, licensed clinical social workers, and field staff to improve outcomes.
• Regularly monitor and adjust care plans based on progress.
• Maintain precise documentation of patient interactions and interventions.
• Engage in case conferences, team meetings, and quality improvement initiatives.
• Participate in daily huddle meetings.
