Qualified candidates should be a licensed RN in MD, VA, and Washington DC, with 5 years of experience in Care Management or direct patient care. CCM certification is preferred, along with strong engagement and coordination skills.

Qualifications

• Registered Nurse with State Licensure in MD, VA, and Washington DC.
• 5 years of clinically related experience in Care Management or direct patient care.
• Certified Case Manager (CCM) or commitment to achieve certification.

Responsibilities

• Research and analyze member's medical and behavioral health needs.
• Develop comprehensive plan of care with members and care teams.
• Monitor and update member's plan of care over time.
