Job summary

Maryland Care Management, Inc. (MCMI) seeks an experienced Care Manager to plan, develop, implement, and oversee care plans for high-risk members.

You will coordinate services, assess health/social needs, and drive timely interventions to improve outcomes. As part of a flexible team, you will interview members, collaborate with physicians and community resources, document plans, monitor progress, and educate providers to ensure quality, cost-effective care.

Qualifications

• RN license active in the state
• Experience with clinical nursing in acute or community settings
• Care management experience is preferred
• Knowledge of managed care programs is a plus

Responsibilities

• Assist in developing and monitoring care management processes for high-risk members
• Collaborate with physicians, social workers, and community resources
• Conduct member interviews to identify risk factors and care needs
• Maintain accurate care management records and ensure data integrity
• Educate medical providers about members’ needs and treatment plans
• Coordinate with social services to promote continuity and quality of care
